Our team will meet with you to teach you about assisted living and determine what your goal is along with a timeline. From there we will do a care assessment to determine what facility licensing would be appropriate for you. Facilities in Wisconsin can be paid for by multiple different sources from private pay, Medicaid waiver programs, long-term insurance, and veteran benefits, we will review these options with you as well as put a long-term plan together. Based on your goals, preferences, and the care assessment results, we will present facility options for you to review. Once you have reviewed the options and approved, we will reach out to each facility to setup an in-person tour. One of our team members will attend the tour with you to ensure all questions are asked and answered. Once a decision is made, we will assist you with getting all appropriate documents and steps completed to be able to set a move-in date. Once you are moved into your new home, we will continue with you by ensuring your questions or concerns are answered for the following 30 days.
